To help you make up daily points you have lost either through illness, injury, or other circumstances beyond your control, you may substitute another activity to help fulfill the requirements. Below we have listed a variety of activities from which you may choose. Your options however are not limited to those listed but if your activity is different from those listed it must be pre-approved by your P.E. teacher. To obtain credit, fill this form out completely and obtain verification of the activity by your parent or Guardian. The requirement is that for each period of Physical Education to be made up you will substitute an activity which takes place a minimum forty minutes. You may make-up only one period per day. Team athletic practice will not be accepted.
